The normal turnaround time for receiving copies of records is 30 days from when payment is received. If you need the information in a shorter time or urgently, you may contact our medical records department by calling between 7:30 AM – 4:00 PM CST, and they will do their best to accommodate your specific needs. They offer expedited medical records requests for an additional fee. This applies to all requests that must be submitted within one week of payment.

To obtain a copy of your medical record, you must print out the authorization, fill it in and return it to us by post, e-mail or fax. The fax number and address for returning the authorization can be found at the bottom of the form.
